• The loyalty card program is available exclusively to patients whose health insurance is affiliated with Adam Vital Hospital.
• The loyalty card should be presented during the first visit or upon registration in the hospital when availing of the program.
• The loyalty card is only valid for one person and non-transferable once registration is initiated.
• The offers are not valid to be clubbed with any other offers outside this program.
• The offers in this program cannot be exchanged for services equivalent to or less than this program given.
• The Pharmacy & Consumables are not included in the 50% discount Outpatient cash services.
